Our Board Certified providers are able to treat a wide variety of medical problems and we believe in old fashioned values when it comes to your medical care. As a patient in our practice, you can be assured that you will be cared for by a provider who knows you and treats you like a person, not a number. Our friendly and compassionate medical team will take the time to listen to your concerns, explain your diagnosis, and answer all of your questions. We also have Admitting Privileges at Christ Hospital and Little Company of Mary Hospital. This allows for same day admission for patients who are in need of further treatment and also offers the patient continuum of care.

Thomas Joseph opened Park Primary Care in 2008 and it is located in the same building as Evergreen Care Center, a Immediate Care facility he opened in 2003. He takes great pride in providing his patients continuous care in an integrated and comprehensive manner.

George Thomas has been with the practice since 2012 and is board certified in Family Medicine. He takes great pride in providing his patients continuous care in an integrated and comprehensive manner.

Dr. Kristina Johnson has been with the practice since 2015 and is board certified in Family Medicine and Sports Medicine. She is passionate about disease prevention and promoting a healthy lifestyle.
Her interests include: Women’s Health (including menopause, thyroid disorders, routine gynecology, menstrual irregularities, etc.) Joint Injection and Sports/Outpatient Orthopedic Care.

Rachel joined our practice in 2016. She demonstrates not only a great enthusiasm for medicine, but a dedication to creating lasting patient relationships and providing top quality healthcare for the entire family.
Her interests include: Preventative Medicine, Women’s Health, Patient Education, Diet/Exercise Counseling and the management of Depression and Anxiety.

Connie joined our practice in 2021. Connie strives to create an environment where her patients feel comfortable and well taken care of. She is efficient, decisive, and has a keen eye for detail. Connie also enjoys the continual state of learning that medicine demands. Staying current with new research is a challenge, but it’s one that she finds very rewarding.

Her interests include: Preventative Medicine, Women’s Health, Weight Management, and Managing Mental Health Conditions (Anxiety, Depression, Eating Disorders)

“I have always been interested in health & wellness. My participation in exercise research, along with studying health behavior change and psychology, has given me a better picture of what it takes for an individual to not only be physically healthy, but to acquire complete physical, mental, and social well-being. Thus, I have a strong belief in preventative care and its ability to prevent future illness in patients. My goal is to provide awareness to my patients in a relaxed environment so that they aren’t afraid of going to the doctor’s office. I also love that every day is different and that I get to meet and build relationships with all kinds of people.”
