We are a primary care office committed to the health and well-being of our community in Evergreen Park, IL. Park Primary Care was founded by Dr. Thomas Joseph in 2003 on the on the belief that a primary care provider should be a “go to” resource for all of a person’s health care needs. We believe your relationship with your provider is designed to be long-term and ongoing in order to provide continuity care and allow you to make the most informed decisions about your health care needs. At Park Primary Care we place a strong emphasis on preventative care, health education and appropriate disease management.

Our Philosophy:

Our philosophy is to treat the underlying problem as well as the symptoms. We spend time taking a thorough history to understand the nature of a patient’s complaint. We focus on primary and secondary prevention of diseases to build a healthy foundation for the future of each patient. We are Board Certified family medicine providers that specialize in providing excellent, caring and high quality healthcare to patients and their families. We have the knowledge, expertise, and skills to diagnose your symptoms and provide you with the effective treatments you need.

PLEASE NOTE: Are you an Advocate HMO Patient? As of January 1, 2018, our status with Advocate Health Care will change and Park Primary Care will no longer be in-network providers of the Advocate HMO Plan. In order to continue to be our patient, you would have to pick a different insurance plan or choose to come see us as a Self-Pay patient. If you choose to stay in the Advocate HMO Network Plan, we will most certainly be able to send your medical records to your new provider. We would suggest calling the Customer Service Number on the back of your insurance card to discuss finding a new provider that is in network.
